• Missions Application
• Participant / Medical / Insurance Information Form

Please return the consent forms and letters to Missions Team.

• Parental Medical Consent Form (For all team members under 18 years of age and must be notarized)
• Waiver of Liability and Parental Consent to Travel (For all team members under 18 years of age, signed in two places and notarized)
• Sample Donation / Support Letter for team members.

IMPORTANT: This entire completed packet should be returned by the requested due date. Please advise your Team Leader if additional assistance is needed. Any delay may jeopardize your participation on the trip.
